Positive Perceptions networks with Aetna, Blue Cross Blue Shield, Cigna, Medcost and Humana. For clients who have insurance carriers that are not listed, your visit is considered out of network, and you will receive a superbill at the end of your visit. With your superbill from our office, which you directly mail to your insurance carrier, according to your plan you may receive your reimbursement. Because of the ever-changing ways that insurance benefits provide for behavioral and mental health services, coverage can only be determined when you contact your insurance company. Often, reimbursement is not completely determined until a diagnosis and list of provided services have been processed by the insurance company.
A statement with all of the necessary information and codes is provided so that you may apply for reimbursement directly from your insurance company. Some have obtained reimbursements from their flex or health savings plans when their medical insurance coverage has denied the claim. All costs that are not reimbursed are fully deductible as medical expenses for income tax purposes. Services may be covered in full or in part by your health insurance or employee benefit plan. Please provide your insurance information for pre-verification when you make your initial appointment and bring insurance card to every appointment thereafter. Copayment, deductible and co-insurance amounts are due at the time of service. Please check your coverage carefully by asking the following questions:
- Do I have mental health insurance benefits?
- What is my deductible and has it been met?
- How many sessions per year does my health insurance cover?
- What is the coverage amount per psychiatric visit?
- Is approval required from my primary care physician?
Cash and all major credit cards accepted for payment. We do NOT accept personal checks.
For all clients and/or their legal guardians, a state issued photo I. D. is a mandatory requirement in our practice or your appoinment will need to be rescheduled.
If you do not show up for your scheduled appointment, and you have not notified us at least 24 hours in advance, you will be required to pay the full cost of the visit.
